Letter from Treating/Ordering MD: "This patient has multiple medical problems which have been reviewed in the in the notes of 5/27/03, 3/21/03, 2/5/02, and 2/16/02... The patient comes in now stating that he is continually dizzy and feeling somewhat uncomfortable. The patient walks with stuttered gait and very small steps. Hiss balance is not good...The patient does have a gait disturbance which is quite pronounced. He is not able to take large steps and he does have poor balance...Patient needs a motorized wheelchair because of the upper body weakness, post CVA (stroke)."
